Closing small ventricular septal defects may not be needed. They often close on their own in childhood or adolescence. Repairing a VSD restores the blood circulation to normal. To date, the FDA has approved use of the CardioSEAL Septal Occlusion System with QuikLoadTM for patients with a complex ventricular septal defect of significant size to warrant closure who are considered to be at high risk for standard transatrial or transarterial surgical closure, based on anatomical conditions and or overall medical condition. High-risk anatomical factors for transatrial or transarterial surgical closure include: requiring a left ventriculotomy or an extensive right ventriculotomy; with a failed previous VSD closure; with multiple apical and or anterior muscular VSDs "Swiss cheese septum" ; ; with posterior apical VSDs covered by trabeculae. FDA, 2002 ; Fontan Procedure Infants born with single-ventricle defects often need multiple operations. These include shunts, such as BlalockTaussig B-T ; or Glenn, placing a band on the pulmonary artery, or the Fontan operation. The Fontan operation largely separates the heart into two circulations. This lets oxygen-poor blood go to the lungs and oxygen-rich blood go to the body. The Fontan operation substantially reduces the mixing of oxygen-poor and oxygen-rich blood and produces a normal or near-normal oxygen supply to the body. It also reduces the risk of a stroke or other complications and decreases the workload on the single ventricle. A fenestrated Fontan leaves a hole or opening in the band shunt to allow some mixing of oxygen-poor and oxygen-rich blood. This is done for patients when it is thought that they cannot tolerate the change in venous pressure. To date, the FDA has approved the use of the CardioSEAL Septal Occlusion System for patients with complex single ventricle physiology who have undergone a fenestrated Fontan palliation procedure and the Amplatzer Septal Occluder for patients who need closure of a previously fenestrated Fontan procedure. Transcatheter procedures are generally well tolerated. Serious complications are uncommon and include device embolization, i.e., the device breaks free whereby surgical device retrieval may be required ; and cardiac arrhythmias usually transient ; . Since the Amplatzer occluders contain a nickel alloy, care must be taken when using this instrument in patients with nickel sensitivity.

Psychotherapeutic treatment means activities provided to assist a person in the identification or modification of beliefs, emotions, attitudes, or behaviors in order to maintain or improve the person's functioning in response to the physical, emotional, and social environment. b. The supported community living services are intended to provide for the daily living needs of the consumer and shall be available as needed during any 24-hour period. Activities do not include those associated with vocational services, academics, day care, medical services, Medicaid case management or other case management. Services are individualized supportive services provided in a variety of community-based, integrated settings. 1 ; Supported community living services shall be available at a daily rate to consumers living outside the home of their family, legal representative, or foster family and for whom a provider has primary responsibility for supervision or structure during the month. This service will provide supervision or structure in identified time periods when another resource is not available. 2 ; Supported community living services shall be available at an hourly rate to consumers for whom a daily rate is not established. c. Services may be provided to a child or an adult. The specific support needs must be identified in the Medicaid case manager's service plan, the total costs shall not exceed 70 per consumer per year, and the provider must maintain records to support the expenditures. A unit of service is: 1 ; One full calendar day when a consumer residing in the living unit receives on-site staff supervision for 14 or more hours per day as an average over a 7-day week and the consumer's individual comprehensive plan or case plan identifies and reflects the need for this amount of supervision. 2 ; One hour when subparagraph 1 ; does not apply. g. ACTION NEUROPATHIC PAIN ONTARIO The Vice-Chair: First, we have Action Neuropathic Pain Ontario. Do you want to state your name for Hansard? Dr. Allan Gordon: I'm Dr. Allan Gordon. I'm a neurologist at Mount Sinai Hospital. I deal with neuropathic pain. This is Rachel Weisz, who is a consumer with neuropathic pain. I'm very pleased that we can speak before you this morning, including to my MPP, whom I've never met, but you'll be hearing from us. I represent a volunteer organization called Action Neuropathic Pain Ontario. This is a group of physicians, pain researchers, nurses, physiotherapists and consumers who have neuropathic pain, who wish to change the way neuropathic pain is managed in this province and to improve access to treatment. I'm also a member of the Canadian Pain Society, a special-interest group, and also something called the association of pain management directors. Neuropathic pain is caused by a number of conditions, including shingles, multiple sclerosis, stroke, diabetes, trauma, post-surgery and cancer. It's hard to know how many people have it, but up to a million people in country and perhaps 400, 000 people in this province. It can be quite severe. Because of it, people can't walk. Please note: The pharmacist's judgment and dispensing restrictions, such as quantities allowable, govern certain controlled substances and other prescribed drugs. Federal law prohibits the return of any dispensed prescription medicines and mesalazine.

As part of the OIFP's investigation into the Stavitski matter, Stephen Poggioli pled guilty to an Accusation charging him with Medicaid fraud. Poggioli admitted that between May 1999 and February 2002, he provided kickbacks in the form of cash and over-the-counter medicine to approximately 15 nursing home and assisted living facilities. These kickbacks were provided in exchange for the facilities' agreement to exclusively use pharmacies owned by Michael Stavitski to provide medications for the facilities' Medicaid patients. On October 24, 2003, Poggioli was sentenced to three years probation. D. Group Therapy Individuals can discuss memories of the trauma, symptoms, and problems of functioning, helping the participants to realize that their actions and emotions are common. 28 ; E. Psychodynamic Therapy This focuses on the individual examining his emotional conflicts. He achieves a greater sense of self-esteem, develops effective ways of thinking and coping, and learns to deal more successfully with intense emotions. 29 ; According to Online Psychological Services, exposure therapy and or EMDR is in many cases the treatment of choice, and is reportedly successful. According to the National Center for Post-Traumatic Stress Disorder, the most successful therapies are CBT and medication. They report that CBT combined with exposure therapy and cognitive restructuring are very effective. OPS also states, "No drugs are currently designated for the treatment of PTSD. Although psychotherapy is commonly used to treat the disorder, its effectiveness is unproven." The NCPTSD states, "There is no definitive treatment, and no cure." The American Psychiatric Association asserts that not everyone who experiences trauma needs treatment i.e. professional psychological treatment ; ; some recover by being helped by family, friends, and clergy. A current trend is to approach PTSD prophylactically. Some studies show that when people are given an opportunity to talk about their experiences right after the traumatic event, some of the symptoms may be reduced. In Hawaii, a study of 12, 000 schoolchildren who experienced and survived a hurricane showed that those who received counseling soon after the hurricane were doing much better two years later that those who did not. The National Institute of Mental Health funds RAPID Grants, so researchers can immediately go to disaster scenes to study the effects of the trauma and to study the effectiveness of early intervention. 30.
